Forms

Build forms and surveys in Realm to collect sign-ups, interest, and feedback from your congregation, and review every response in one place.

Many churches gather sign-ups, waivers, and feedback with outside form tools, then copy the answers into their records by hand. With Forms, you build the form, share it, and review the responses in a .csv file you download from within Realm. People don't have to sign in to fill out a form, so guests can respond too. When someone is signed in, their answers are saved with their name and email in a .csv file.

For example, a Fall Festival form can ask what candy each family is bringing for trunk-or-treat and which hour they can help at a game station.

Forms or Registration Questions

Registration events have their own registration questions, so it helps to know which one to use.

  • Registration questions work best for collecting a few details while someone registers for an event. For example, when a parent registers a child for camp, registration questions can ask for the child's T-shirt size and food allergies.
  • Forms work best for longer forms and surveys, such as a waiver for camp activities or a feedback survey after an event. You can attach a published form to an event. When staff checks the event, they'll see a warning listing registrants who have not completed the form.

What's in a Form?

You design your form or survey under Events & Forms. Each form has these parts:

  • A public title, which is the only required field.
  • An image and description that tell people what the form is for.
  • An optional target group that ties the form to a ministry area, a group, or a system group such as your entire church.
  • An optional end date, after which the form closes.
  • Questions. Choose from question types such as short text, multiple choice, yes/no, and date. Group questions into sections, and add follow-up questions that appear only when someone picks a certain answer.

How a Form Moves from Draft to Closed

  1. Create a new form or copy an existing one: A new or copied form is a draft. Only people who can use Forms see it.
  2. Preview. Before you share, preview the form and answer it the way a member would. It's the easiest way to catch a typo, a missing answer choice, or an image that crops badly.
  3. Publish and share. When you publish the form, its status changes from Draft to Shared and people can open it. Then send out a link or a QR code, and choose whether people must sign in to answer.
  4. Collect responses. The Forms list shows how many responses each form has. To see the responses, download them as a .csv file. If someone who's signed in fills out the form again, their new answers replace their earlier ones.
  5. Close. When the end date passes, the form moves from the Current tab to the Past tab.

Viewing Responses

Responses are written to a .csv file per form. To download this file, click the more options button to the right of the form, then click Download CSV. You can download responses for current and past forms.
