Create Forms

Start a new form by giving it a title, a description, and an optional end date. The form stays a draft until you publish it.

To build or manage forms, you need the Manage Forms permission set to Allow in your responsibilities. Administrators have this permission by default.

Forms can be used with or without events. When used in conjunction with events, you can indicate one or more forms to send to the person who registered for themselves and others, or to send to each registrant. You can also create and manage forms for use without events. When building a form, add an image and questions before you save, or come back and add or revise them later. Your work is always saved.
  1. In the top-left corner, click your site name then Realm. Then click Events & Forms > Forms.
  2. Click New Form.

  3. Enter a Form Title (Public). People see this title at the top of the form, and it's the only field you need before you can save.
  4. Enter a Description that tells people what the form is for and anything they need to know before they answer.
  5. To tie the form to a ministry area, a group, or a system group such as your entire church, select it in Form Target Group.
  6. To close the form on a certain date, select Set an End Date, then choose the End Date.
After the end date passes, the form moves from the Current tab to the Past tab.
  7. Click Save.
The form appears in the Forms list with a Draft status, and the Preview and Publish buttons appear.

Next, add an image and revise or add more questions, then preview the form before you publish and share it.

To send the form to people who register for an event, publish it, then add it as a follow-up form on the event's Questions and Forms tab. For steps, see Attach a Follow-up Form to a Registration Event.
