Using the Prayer and Feedback widget, people can submit a prayer request, praise report, comment, and more! If needed, people can mark prayer requests as confidential with the check of a box.

• Those who log in can submit the prayer request or feedback as themselves, anyone else in their household, or from a blank form. Those who don't log in will be asked to provide their contact information.
• Feedback types are controlled by optional attributes, so you can customize the experience. We suggest having one Prayer and Feedback widget limited to prayer requests and praise reports and a second Prayer and Feedback widget to gather comments.
• After submitting their prayer request or feedback, the person receives a verification email with a one-time link to verify their feedback.
• MinistryPlatform creates the prayer and feedback record after the submission has been verified.
• When the Private checkbox is selected, the submission's visibility will be set to 2 - Staff Only.
• If the Prayer Request and Feedback widget is filtered by a program ID, MinistryPlatform records that program on the prayer and feedback record.

Contact Information

Those who are not logged in will be asked to provide their contact information (first name, last name, email, and, optionally, phone number) so MinistryPlatform can match their feedback with the correct contact.

Provide Feedback As

Those who are logged in can submit a response as themselves, anyone in their household, or from a blank form. If they submit from a blank form, they'll be prompted to provide their contact information.

Feedback Type

The type of feedback the person provides. Available feedback types are controlled by an optional attribute so you can do your church your way.

Feedback Summary

A required summary of the feedback written by the person.

Feedback Details

The person's prayer request or feedback.

Private

Entries that are marked Private will have a visibility level of 2 - Staff Only.

Actions

Submit

This action triggers a verification email asking the person to verify their prayer and/or feedback.

Verification

When the person clicks the link in the verification email, MinistryPlatform creates a prayer and feedback record associated with the person logged in. If there is no match for the person's first name, last name, email, or phone number combination, it creates a contact record. The email link is for single use only, so once it is clicked, it cannot be used again. This prevents duplicate submissions. A message will confirm that the request has been verified.
