Attach a Follow-up Form to a Registration Event

Send a custom form with the registration confirmation email to collect details you need before the event.

  • Only available to administrators and users who have the Manage Events permission.

  • The form must be published before you can attach it. To build one, see Create Forms and Publish and Share a Form.

Use a follow-up form for details you need before the event but don't need at registration, such as a medical release, photo consent, or a packing list. You can attach more than one form to an event.
  1. In the top-left corner, click your site name then Realm. Then click Events & Forms > Registration Events > Upcoming.
  2. Open the event you want to send the form with, or click Add Event to create one.
  3. Click the Questions and Forms tab.
  4. Under Follow-up forms, click Add Follow-up Form.
  5. In Form to send, select the form.
Only published forms are listed. If you don't see the form you want, click Manage custom forms to build or publish it. You see this link only if you have the Manage Forms permission.
  6. Under Send to, choose who gets the form:
    • The person who registered gets one email and fills out the form for each person they registered.
    • Every registrant gets their own email and fills out their own form. For registrants without an email address, the email goes to the person who registered.
A line at the bottom of the form box confirms which form is included in the confirmation email and who gets it.
  7. To send another form, click Add Follow-up Form again and repeat the last two steps.
  8. Click Save Event.
The form is included in the registration confirmation email. Each follow-up form on the event shows how many people have responded.
To get the answers for this event, click Download CSV on the follow-up form. To stop sending a form, click Remove on it, then save the event. For more about building forms, see Forms.
