Build your own forms and surveys in Realm, share them with a link or QR code, and send follow-up forms with registration confirmation emails.

Build Your Own Forms and Surveys

You can now build forms and surveys right in Realm. Give a form a title, image, and description, then add questions such as short text, multiple choice, yes/no, and date. Group questions into sections, and add follow-up questions that appear only when someone picks a certain answer. See Forms.

  • Bring sign-up sheets, waivers, and feedback surveys out of outside form tools, so your responses stay with the rest of your church's information.

  • Preview a form the way your congregation will see it, and catch a typo or a missing answer choice before anyone else does.

  • Set an end date so a form closes on its own, and copy a form to reuse it next year.

Administrators can build forms by default. To let other staff build and manage forms, give them the Manage Forms permission.

Preview your form before publishing and sharing.

Share a Form and Collect Responses

When a form is ready, publish it and share it with a link or a QR code. Paste the link into an email, a text, or your website, or print the QR code for a bulletin or a lobby sign. See Publish and Share a Form.

  • Guests can fill out a form without signing in. When you need to know who responded, turn on Require a Login.

  • See how many responses each form has in the Forms list, and download them in a spreadsheet when you're ready to follow up. See Download Form Responses.

Share the link to a published form or require a login.

Send Follow-Up Forms with Event Registrations

You can also attach a published custom form to a registration event, and it's sent with the registration confirmation email. Use a follow-up form for details you need before the event but not at registration, such as a medical release, photo consent, or a packing list. On the event, the Custom Questions tab is now called Questions and Forms. See Attach a Follow-up Form to a Registration Event.

  • Keep registration short. Ask only what you need to register, and collect the rest afterward.

  • Choose whether the person who registered fills out the form for everyone they registered, or each registrant gets their own.

  • See at a glance who still owes a form. The registration shows a Missing Information alert until every registrant has completed it, and staff can fill out a form for someone who calls in or turns in a paper copy.

  • See how many people have responded on the event, and download only that event's responses.

On the same page as registration questions, attach a form or survey to the registration process.
