If you're a small employer with self-insured health care coverage, prepare Form 1095-B.

1. In the top-left corner, click your ministry hub then Accounting. Then click Period End > Tax Forms.
2. On the Tax Forms tab, select the reporting year.
3. Click Prepare Forms beside 1095-B.
4. Select the origin of the policy. For more information on what these mean, visit the official IRS website.
5. Click Save.
